Helping to care for frail older people outside of hospital

Frailty and ageing often go hand in hand. As we get older, we become at higher risk of adverse outcomes. Falls, mobility decline, admissions, longer lengths of stay or the need for long-term care. But how can digital technology help?

With the support of CliniTouch Vie, you can discharge your frail patients as soon as they’re ready to leave hospital. Whether that’s to their own home or a care home, our technology provides the support they need. And once they’re safely at home, we can help to make sure they stay there too. CliniTouch Vie reduces readmissions and improves outcomes. Plus, it helps to give older people their independence back.

Frailty is a major clinical focus for the rollout of virtual wards. We can help you to launch and deliver brilliant virtual care.

The impacts of deploying a digital platform

We conducted an evaluation to assess the impact on care quality and resource use in deploying our digital CliniTouch Vie platform. Focusing on residents with frailty in a care home setting, we found that the use of digital is cost saving for practices whilst improving care quality – even within the context of a pandemic.
